Blepharoplasty & Midface Lift
My Approach
Rejuvenating the eyes isn’t just about removing excess skin—it’s about restoring structure and support. In many middle-aged patients, especially men, aging around the eyes comes from volume loss, fat descent, and weakening of the lid–cheek junction rather than simple skin redundancy. If we only tighten or excise tissue, the result can look hollow or overdone. For this 61-year-young male, I used an individualized, multi-layered approach focused on repositioning rather than removing. Through the lower eyelids, I performed a vertical midface lift with ligament release and suspension of the superficial fat pads to restore natural cheek support. The orbital fat was conservatively reset and transposed to the SOOF to smooth the lid–cheek transition—no fat removal at all. Upper and lower blepharoplasty were performed with minimal skin excision, along with a direct brow lift tailored specifically to the male hairline to avoid feminization. Targeted fat grafting restored volume to the upper sulcus and midface, and fractional CO₂ resurfacing improved skin texture and sun damage. The goal was balance and strength, not tightness—restoring harmony without changing identity.

The Healing Process
Early swelling and redness are expected during the first one to two weeks, particularly around the lower lids and brow incisions. Most patients notice steady improvement by the one-month mark, though subtle inflammation can persist as tissues continue to settle. The brows may initially appear slightly elevated but naturally relax into a more neutral position over the following weeks. Because this technique avoids more aggressive maneuvers, recovery is generally smoother, with minimal downtime compared to traditional facelift procedures. By week three, most patients feel comfortable resuming normal activities. Facelift results settle within a few months, while the nose continues to refine for up to a year. Patients are encouraged to sleep elevated, avoid strenuous exercise, and follow postoperative instructions closely, which helps minimize bruising, supports healing, and ensures more predictable, long-lasting aesthetic outcomes overall satisfaction levels.
FAQs
Blepharoplasty improves droopy upper eyelids, under-eye bags, puffiness, and excess skin. It helps create a more rested, alert, and youthful appearance without changing your natural eye shape.
A midface lift repositions descended cheek tissue upward to restore volume beneath the eyes and improve the lid–cheek junction. It reduces hollowing, softens deep grooves, and provides better lower eyelid support.
A facelift mainly addresses the jawline and neck. Blepharoplasty and midface lifting focus on the central face—the eyes and cheeks—where early aging is often most visible. Many patients benefit more from this targeted approach.
In most cases, no. Removing fat can create a hollow or aged look. Repositioning or redistributing fat typically produces smoother, more natural contours.
Patients with under-eye bags, hollowness, sagging cheeks, or a tired appearance despite good skin quality are often excellent candidates. It’s also ideal for those who want improvement without a full facelift.
Most patients return to work and social activities within 10–14 days. Swelling improves steadily over several weeks, with final refinement visible over a few months.
